Abstract Aims Lost spilled gallstones during laparoscopic cholecystectomy is a rare inadvertent complication results in recurrent delayed sepsis. Diagnosis made via retrospective review of operative notes. 2 cases retained were removed surgically, following several years after the initial cholecystectomy. Conclusion presented with symptoms abdominal pain, sepsis intra-abdominal collections. Intra-abdominal abscesses treated percutaneous USS guided drainage & antibiotics. Abscesses arise right subphrenic space, anterior wall iliacus as result gallstone migration. “spilled lost gallstones” was until confirmation on radiological imaging. Combination USS, CT and MRI scan enabled to detect (radiolucent or radio-opaque) within an abscess cavity. Multi-disciplinary approach allowed more concerted management i.e. diagnosis planning surgical exploration for gallstones. Ultimately, should be drained (percutaneously surgically), stones removed. Ideally this done minimally invasive techniques, but open surgery often required. Use laparoscopy enables peritoneal cavity any This will extract fragment that could nidus continuous infection.
